Please come to the appointment with a filled- out questionnaire, a photo (emailed photo is preferable), and a registration fee of $60 which does not have to be renewed. It is valid until mazel tov time!
If you live at a distance or your schedule makes it difficult for you to meet in person, please download the questionnaire, fill it out, and email it with an attached photo. Please snail-mail the registration fee (checks should be made out to Likrat Shiduch and sent to 5 Nelson Place, Maplewood, NJ 07040). We will call you in about a week from the time we receive it, to give you a telephone interview.
In addition, please sign the Likrat Shiduch Contract and peruse the "Commitment Contract."
